OverviewCare Wisconsin is a nonprofit care management organization specializing in the integration of health and long-term care services. For over 34 years, we have been filling gaps in community services and providing innovative solutions for seniors and adults with developmental and/or physical disabilities. We have roots as a human services agency. With the support of community volunteers, Care Wisconsin started the region's first adult day center in a church hall in Madison in 1976. Today, we manage two of the State's public health and long-term care programs: Family Care and Partnership. Care Wisconsin serves over 4,200 members across 10 counties in the south central region, and has 562 full- and part-time employees.
