Many older adults experience chronic conditions, physical disabilities, cognitive concerns, or perhaps most commonly decline in the ability to carry out their daily activities. Care Wisconsin offers a range of programs and services designed to address these situations:
- The Partnership Program provides coordinated health care and long-term support services for Medicaid-eligible older adults who have chronic conditions requiring nursing home level of care.
- Our two Adult Day Centers are bright and vibrant places where older persons with medical, physical, or cognitive disabilities can actively and enjoyably spend the daytime.
- Home Care Services assist older adults with bathing, getting dressed, meal preparation, home chores, respite, and other care, so they can continue to live at home.
Over the past three decades, Care Wisconsin has evolved and expanded to meet the changing needs of a diverse, growing aging population and shifting economy. Care Wisconsin, a private nonprofit organization, is a nationally recognized leader in health care management and innovative high-touch care for older adults with severe and disabling chronic conditions.
Today, Care Wisconsin has over 275 health professionals, direct care workers, support staff and volunteers serving more than 650 older adults and 1,100 caregivers each year.
