Care Wisconsin exists to provide health care coordination and support services to older adults with chronic medical conditions, physical disabilities, cognitive challenges, and/or decline in their abilities to function at home.
Our approach is holistic, considering the needs of the "whole" person within the context of each unique situation. We always keep dignity and personal wishes at the forefront of the care plan that is created for each individual.
We do this by partnering with the older adult, family members, local doctors, and other service providers in the community. Together, we aim to:
- Foster independence and help keep older adults at home or in the community for as long as possible;
- Apply preventive measures, by anticipating problems and concerns in advance;
- Provide direct assistance, advice, and respite for family and other caregivers;
- Improve and maintain quality of life.
Many older adults experience chronic medical conditions, physical disabilities, cognitive concerns, or - perhaps most commonly - decline in the ability to carry out their daily activities. Partnership Program
Provides coordinated health care and long-term support services for Medicaid-eligible older adults who have chronic conditions requiring nursing home level of care.
Adult Day Centers
Bright and vibrant places where older persons with medical, physical, or cognitive disabilities can actively and enjoyably spend the daytime.
Home Care Services
Assist older adults with bathing, getting dressed, meal preparation, home chores, respite, and other care, so they can continue to live at home.
