Dental Hygienist
1 Part Time (.4 FTE) position (Tues. and Thurs., 8:00am-4:30pm)
Under supervision of the Administrative Services Manager and the guidance of the Care Wisconsin  Dentist, the Dental Hygienist position is responsible for providing participant dental care, assisting in both development and implementation of an overall agency dental program, providing dental assistance to dental residents and dentists.. It is an expectation of this position to maintain complete confidentiality in matters related to both staff and participant information.

Requirements:

  • Minimum of a two-year Dental Hygienist Degree required.
  • Licensure or eligibility for licensure in the State of Wisconsin as a Dental Hygienist.

Preferred:

  • Knowledge of physical, mental and social needs of the frail elderly population.
  • Experience in active participation in the administrative details of a dental office.

RN Practice Leader - Partnership
1 Full Time (1.0 FTE) position (M-F, 8:00am-4:30pm)
Under the general direction of the Chief Medical Officer, in conjunction with the Medical Director, and in collaboration with other members of the Clinical Administration Team (CAT), the Registered Nurse (RN) Practice Leader position will oversee practice, play a key role in staff hiring and performance support, and provide oversight within their discipline.  This position will participate in the day-to-day leadership of the Partnership Program. 

Based on census growth (which could require adding new teams) or changing operational needs, actual percentage of time spent for any particular responsibility may vary from the percentage(s) listed below. This position may spend varying amounts of time on the below-mentioned duties based on need in those certain areas. 
 
Requirements:

  • Minimum of two years experience as a professional Partnership Team member.
  • Leadership, organizational, negotiation, judgment, problem solving, and decision-making skills.
  • Possession of a valid, State of Wisconsin driver’s license.

RN Care Manager – Home Care
1 Full-Time (1.0 FTE) position (M-F, 8:00am-4:30pm)
Under the general supervision of the Education Specialist, and in close collaboration with the Home Care Manager, the Nurse Care Manager – Home Care position is responsible for bridging communication, and assuring collaborative efforts among departments (Home Care, Partnership Teams, Transportation, Day Centers, Outreach & Enrollment) to assure the needs of members requiring Home Care services are being met in a coordinated, seamless fashion.  This position provides nursing assessments as necessary, particularly around members with high Home Care needs and participates in care planning with the Interdisciplinary Team.  This position is also responsible for representing Home Care’s scope of service to other departments in a consistent, systematic way and to assure Home Care services are provided in the home setting using principles and processes that promote safety of the member and safety of the staff.  The Nurse Care Manager – Home Care works closely with the Education Specialist, the Home Care Manager and the Home Care Practice Leaders to meet the Home Care needs of members and to meet the orientation and training needs of Home Care staff.  The incumbent in this position is part of the Home Care leadership team and is expected to participate in developing and implementing departmental goals.


Requirements:

  • Associate’s Degree in Nursing.
  • Licensure or eligibility for licensure in the State of Wisconsin as a Registered Nurse (RN).
  • Knowledge of the human aging process and geriatric nursing issues.

Home Care Administrator
1 Full-Time (1.0 FTE) Position (M-F, 8:00am-4:30pm)

Under the direction of the Director of Program Operations, the Home Care Administrator position is responsible for operations and resource planning, fiscal and budgetary oversight, evaluation of services, support of new program development and implementation, and direct oversight of the Home Care department.  The Home Care Administrator plays a primary role in ensuring good integration of operational activities between clinical programs, the Home Care department, and other departments, as appropriate, within Care Wisconsin.  The Home Care Administrator is also responsible for evaluating home care services being provided by contracted providers, understanding the home care needs of Care Wisconsin members in all programs and identifying new ways to meet those needs in creative, cost effective and efficient manner. 

Requirements:

  • Minimum of five years experience in planning, program operations, budgeting and personnel management in a clinic, hospital, service agency, or similar setting.
  • Bachelor’s Degree in Nursing.
  • Possession of a valid, State of Wisconsin driver’s license.
  • Knowledge of and ability to use computer applications as used in the analysis of data, writing, and presentation of information.

Preferred:

  • Master’s Degree in Nursing, Business, or related area.

Family Care Team Development Specialist- North Central Region
1 Full-Time (1.0 FTE) position (M-F, 8:00am-4:30pm)

Under the general supervision of the Regional Director of Long Term Care Programs, this position is responsible for developing interdisciplinary team development activities for the purpose of strengthening relationships and communication skills for the Family Care teams and for other Care Wisconsin programs as needed. This position will act as a team facilitator for assigned teams to provide guidance to the teams in determining participant care coordination. This position will assist in creating a climate of shared values, participant satisfaction, respect, innovation and the promotion of excellence.

Requirements:

  • Master’s Degree in Social Work from an accredited school.
  • Minimum of two years experience working with the elderly or adults with disabilities.
  • Experience with computer applications as used in the analysis of data, writing, and presentation of information.
  • Possession of a valid, State of Wisconsin driver’s license.

Preferred:

  • Certification as a Licensed Clinical Social Worker (LCSW).
  • Experience and/or training leading team building activities.
  • Experience working in a care management setting for long term care services.

Phlebotomists

Per Diem position (hours vary)

This position is responsible for drawing blood from participants for analysis or other medical purposes. This position is also responsible for learning the lab requirements used for each lab and verifying that paperwork is filled out correctly. This position does require the ability to navigate throughout Dane County to participants’ homes by following route schedule and allotted times for draws.

Requirements:

  • High School diploma or GED; or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Effective customer service skills.
  • Valid State of Wisconsin driver’s license, auto insurance and reliable transportation.

Preferred:

  • Minimum of one year phlebotomy experience.
  • Minimum one year experience working with the elderly or adults with physical and developmental disabilities.

Pharmacy Services Manager

1 Part-Time or Full-Time (0.6-1.0 FTE) position

Under the general supervision of the Chief Medical Officer, this position is responsible for drug utilization evaluation and Medication Therapy Management of medications prescribed for Care Wisconsin Health Plan members.  This position integrates best practice guidelines in the management of Care Wisconsin formulary, Prior Authorization (PA) and step therapies. This position is responsible for: managing and updating the drug formulary according to regulations set by CMS; monitoring the Part D regulatory requirements of CMS for Care Wisconsin Health Plan; proactively looking for opportunities to improve outcomes; conferring regarding drug efficacy and cost containment of drug therapy; performing comprehensive assessments and analysis in collaboration with interdisciplinary team members in meeting the healthcare and educational needs of the members and staff; and taking a leadership role for cost control and quality assurance purposes.

Requirements:

  • Doctor of Pharmacy degree or equivalent degree required.
  • Possession of a current licensure as a Wisconsin Registered Pharmacist.
  • Minimum of two years of professional level experience in a health care setting, including one year of formulary experience.

Preferred:

  • Knowledge of Medicare/Medicaid regulations and administrative guidelines.
  • Experience in long term care, pharmacy management, and/or managed care experience.
  • Previous leadership experience.

Physician Assistants (PA)-Partnership

Full-Time (1.0) FTE positions (M-F, 8:00am-4:30pm)

Under the general supervision of the Medical Services Manager, the Physician Assistant (PA) is responsible for managing member care through collaboration with primary physicians to provide advanced assessment, diagnosis and management or oversight of acute and chronic illness and primary health care. 

 Requirements:

  • Completion of an accredited Physician Assistant training program.
  • Possession of Drug Enforcement Administration (DEA) registration (or eligibility).
  • Possession of a licensure (or eligibility for licensure) in the State of Wisconsin as a Physician Assistant.
  • Knowledge and skill in advanced physical assessment.

Registered Nurses-Partnership  ***$2500 Sign-on Bonus***

Full-Time (1.0 FTE) positions (M-F, 8:00am-4:30pm)--Dane and surrounding counties

Under the general supervision of the Registered Nurse (RN) Practice Leader, the Registered Nurse position is responsible for assessing, planning, intervening/delivering, coordinating, monitoring and evaluating in-home care for participants in collaboration with the care team to enable participants to live as independently as possible in the community.  The RN provides a range of services which include: providing participant care; coordinating and monitoring care management in collaboration with interdisciplinary team; delegating and supervising in-home health workers and various facilities staff; participating in discharge planning; and related administrative duties. Excellent interpersonal communication, problem solving and conflict resolution skills are essential.  The RN must be able to function with a high degree of independence while actively working in collaboration with other team members.   The ability to work autonomously and use organizational skills is essential to successfully provide participant services as well as to respond to requests from physicians, other health care disciplines and community professionals.

 Requirements:

  • Licensure as a Registered Nurse in the State of Wisconsin.
  • Over one year of experience as an RN —we prefer previous acute care and/or home health experience.

Care Managers-Family Care

Full-Time (1.0 FTE) positions (M-F, 8:00am-4:30pm)--Waukesha, Dodge, Jefferson, Tri-County, and Columbia counties
The Care Manager’s, along with the Family Care Team, will be responsible for assessing, planning, intervening/delivering, coordinating, monitoring, and evaluation services for members in order to enable members to live as independently as possible. The Care Manager coordinates a range of member centered initiatives which include: member advocacy, care planning and provider relationships. The program goal is to promote the overall well-being of members by facilitating high quality, member centered outcome based services as part of the interdisciplinary team. Services are coordinated for people who are frail elderly, adults with physical disabilities and adults with developmental disabilities.

 Requirements:

  • Bachelor degree is Social Work or related Human Services field.
  • Knowledge of physical, mental and social needs of adults with a variety of physical and mental health conditions.
  • Ability to review and analyze data and make recommendations based on collected information.
  • Knowledge and ability to execute a goal-oriented care plan and experience in completing comprehensive assessments.
  • Ability to work additional hours or flexible hours and participate in an on-call rotation as necessary.
  • Valid, State of Wisconsin driver’s license in good standing (or the ability to obtain) and reliable transportation.

 Preferred:

  • Social Work certification.
  • Minimum of one year experience in care management.

 Requirements:

  • Associate degree in Nursing.
  • Licensure or eligibility for licensure in the State of Wisconsin as a Registered Nurse.

Physical Therapist   ***$1500 Sign-on Bonus***

1 Part-Time (0.5 FTE) position

Under the general supervision of the Administrative Services Manager, the Physical Therapist is responsible for evaluating and treating participants to enable them to live as independently as possible in the community. The incumbent works within the scope of federal and state regulations as well as state specific practice acts.  A significant amount of collaboration with medical, nursing, rehabilitation and other staff is required.

Requirements:

  • Certification or eligibility for certification as a Physical Therapist in the State of Wisconsin, Department of Regulation and Licensing.
  • Master’s Degree in Physical Therapy from an accredited college or university required
  • Ability to frequently handle and transfer participants of over 100 pounds.
  • Ability to repetitively push, pull and grasp up to 100 pounds on an occasional basis.
  • Ability to be flexible and to work additional hours if necessary.
  • Ability to work well both in a team environment and independently.

Preferred:

  • Minimum of one year experience as a Physical Therapist.
  • Previous experience in long term care or geriatric rehabilitation.
  • Possession of a valid, State of Wisconsin driver’s license in good standing (or the ability to obtain) and reliable transportation.
