Joining Our Network

Care Wisconsin First, Inc. and Care Wisconsin Health Plan, Inc. have contracts with Wisconsin Medicaid and Medicare. Care Wisconsin is a state-certified care management organization authorized to manage health, medical and long-term care services under Wisconsin's Family Care and Partnership public programs.

Care Wisconsin members must obtain services from network providers. Our goal is to have a broad range of quality providers to meet the service needs of Care Wisconsin members in the service area, including Columbia, Dane, Dodge, Green Lake, Jefferson, Marquette, Sauk, Washington, Waukesha and Waushara counties. "Providers" is a general term used for health, medical and long-term care service providers that are licensed and/or certified by Medicare and the state to furnish services. Network providers have signed a contract to provide services to members and agree to follow Care Wisconsin policies, procedures and guidelines.

We ensure the integrity of our network providers through credentialing, utilization review and ongoing quality initiatives.

For over 32 years, Care Wisconsin has been partnering with local providers of services for seniors and adults with physical and/or developmental disabilities to maintain quality of life and independence. This mission-driven nonprofit organization opened the state's first adult day center in 1976 and today continues to be a leader in managing community-based services.
