Nurse Care Managers (RN)-Family Care Full-Time (1.0 FTE) positions (M-F, 8:00am-4:30pm) available in Waushara county The RN Care Manager will be responsible for assessing, planning, intervening/delivering, coordinating, monitoring and evaluation services for members in order to enable members to live as independently as possible. The RN, in conjunction with the Family Care Team, will oversee a range of services, which include: member care, delegation and oversight of care to home health agencies, participating in discharge planning, and related administrative duties. Services are coordinated for people who are frail elderly, and adults with physical and developmental disabilities. Requirements: - Associate degree in Nursing.
- Licensure or eligibility for licensure in the State of Wisconsin as a Registered Nurse.
- Ability to review and analyze data and make recommendations based on collected information.
- Knowledge and ability to execute a goal-oriented care plan.
- Ability to review and analyze data and make recommendations based on collected information.
- Basic keyboarding skills.
- Valid, State of Wisconsin driver’s license in good standing (or the ability to obtain) and reliable transportation.
- Ability to work additional hours or flexible hours and participate in an on-call rotation as necessary.
- Ability to effectively work with a team to manager member financial resources.
Preferred: - Bachelor degree in Nursing.
- Experience in a community based or long term setting.
- Experience as a member of an interdisciplinary team providing nursing care.
- Knowledge of community nursing, nursing home level of care definitions and health and long-term care issues.
- Knowledge and experience completing nursing and health assessments.
- Microsoft Office Suite (Word, Excel).
