Joining Partnership
How Can You Become a Member?
You must be eligible for Wisconsin Medicaid.
If you are a Dane County resident and want to enroll in Partnership, you must:
- be age 18 or over and have a physical disability; or
- be age 65 or over;
- have long-term care needs;
- meet functional and financial requirements; and
- be a resident of Dane County.

If you are a Columbia, Dodge, Jefferson or Sauk County resident and want to enroll in Partnership, you must:
- be age 18 or over and have a developmental and/or
physical disability; or - be age 65 or over;
- have long-term care needs;
- meet functional and financial requirements; and
- be a resident of Columbia, Dodge, Jefferson or Sauk County.
