Do you need long-term care plus health and medical benefits? Partnership may be right for you.

Partnership is a public program that provides expanded care management and covers all long-term care services—plus:

  • ALL health and medical services, and
  • ALL prescription drugs.

You, your family or your guardian will have the extra support of a care team that is trained to provide care management for both health and medical conditions, in addition to your long-term care needs.

Expanded Care Team
You are the most important member of your care team. In addition to you, your care team includes a nurse practitioner, a registered nurse, a social worker and other professionals.

Your care team will assist in coordinating all your health, medical and long-term care services.

You will experience the advantages of having a nurse practitioner on your care team. The nurse practitioner can prescribe medications, perform general physical exams and help in communicating with your doctor.

Member-Centered Approach
You and your care team will discuss what's important to you and what personal goals or long-term care outcomes you want to achieve. You will determine what you are able to do for yourself, what supports you have already and what your needs are.

Your care team will create a personalized service plan, which will focus on the outcomes you want and the resources you may need to achieve those outcomes. Together, we will put in place the right services in the right amounts at the right time to support your changing needs.

You also have the opportunity to arrange, direct and purchase some or all of your long-term care services through the icon Self-Directed Supports option.
