The Partnership Program is designed to assist people who have chronic health problems or functional decline that prevents them from living in the community without assistance.

To enroll in Partnership, you must:

• be age 55 or older
• live in Dane County, Wisconsin
• be eligible for nursing home level of care as determined by the State
• meet certain financial requirements

Our friendly staff can talk with you on the phone or visit your home to explain the Partnership program. Family members and other caregivers are always welcome to join in the discussion. We will review the eligibility requirements, cover the benefits of the program and answer any questions you may have. This visit is free of charge, and there is no obligation to enroll. Our goal is to provide you with enough information so you can decide if Partnership will meet your health and support care needs.
