Continued independence and quality of life are common desires for ourselves and our loved ones as we grow older. For individuals with chronic health conditions or functional decline, it can be difficult to meet these challenges without assistance. For many older adults with these concerns, Partnership may be the best program to meet these needs. We partner with you, local doctors, family, and the community to create a service plan that emphasizes preventive care and planning in advance. You can receive many health and personal care services right in your home. Your own Care Team - nurse practitioner, nurse, social worker and other professionals - is trained to coordinate and provide care to meet your needs. Most importantly, your Care Team gets to know you very well and works closely to support you in your wish to live independently. You no longer need to worry about getting to your doctor, setting up appointments and dealing with confusing medical bills. Partnership will coordinate all your health care and support services. Medicaid and Medicare provide the funding for eligible participants. Partnership is part of the Wisconsin Partnership Program. We provide you with the health and supportive care you need to remain at home and active in the community.
Eligibility Requirements To enroll in Partnership, you must: - be age 55 or older
- live in Dane County, Wisconsin
- be eligible for nursing home level of care as determined by the State
- meet certain financial requirements
