Care Management Programs

Family Care
Family Care offers Medicaid, home and community-based long-term care services, as well as the support of a local care team. It is a Wisconsin Department of Health Services public program. Care team includes member, a care manager/social worker and a registered nurse.

Partnership
Partnership offers all long-term care services in the basic Family Care package, plus all health and medical services, and prescription drugs. It is a Wisconsin Family Care Partnership and Medicare Advantage Special Needs Plan program. Care team includes member, a care manager/social worker, a registered nurse and a nurse practitioner.

Our Care Model
The care model for both Family Care and Partnership places the member at the center of all care. It is designed to put in place the right services in the right amounts at the right time. By empowering members to be active partners in their service plan, the focus is on achieving personal goals, prevention, continuous monitoring, early intervention, and flexibility.
