Care Management Programs

Family Care Program
Familly Care offers Medicaid, home and community-based long-term care services, as well as the support of a local care team. It is a Wisconsin Department of Health Services public program. The care team includes the member, a care manager/social worker and a registered nurse.

Partnership Program
Partnership offers all long-term care services in the basic Family Care program, plus all health and medical services, and prescription drugs. It is a Wisconsin Family Care Partnership and Medicare Advantage Special Needs Plan program. The care team includes the member, a care manager/social worker, a registered nurse and a nurse practitioner.

Eligibility and Enrollment
The county's Aging and Disability Resource Center is the point of entry where options counselors walk consumers through their different choices, determine eligibility and process enrollment.

During the transition to Family Care and Partnership from the Community Options Program (COP) and Community Integration Program (CIP) in 2008, current waiver program enrollees are automatically eligible. At the same time, those eligible individuals on the waiver waitlist will be enrolled every month through 2010 until everyone is enrolled and the program becomes an entitlement.
