Joining Our Network
Care Wisconsin is a nonprofit managed care organization specializing in publicly-funded programs that integrate health and long-term care services. We offer two of the State's health and long-term care programs through two nonprofit organizations. Care Wisconsin First operates the Family Care Program. Care Wisconsin Health Plan operates the Partnership and holds an HMO license from the Wisconsin Office of the Commissioner of Insurance.
Care Wisconsin members generally must obtain services from network providers. Our goal is to have a broad range of quality providers to meet the service needs of Care Wisconsin members in our service area, including Columbia, Dane, Dodge, Green Lake, Jefferson, Marquette, Ozaukee, Sauk, Sheboygan, Walworth, Washington, Waukesha and Waushara counties.
"Providers" is a general term used for health, medical and long-term care service providers, In many instances, providers are licensed and/or certified by the WI Department of Health Services, other State agencies, or the Centers for Medicare & Medicaid Services (Medicare).
Network providers have signed a contract to provide services to members and agree to follow Care Wisconsin policies, procedures and guidelines. We ensure the integrity of our network providers through credentialing, background checks, utilization review and ongoing quality initiatives.
Members in our Family Care and Partnership may also choose to self-direct some or all of their long-term care services. If a member chooses the Self-Directed Supports (SDS) option, service brokers and fiscal intermediaries may assist in engaging, credentialing and paying providers.
