Sharing StoriesThere’s the story, and there are the stories behind the story. By reading about our members’ experiences, you can learn how we partner with others in the community to make their lives better. For instance, read the story of 52-year-old Dane County Partnership member Cheryl Depp who was diagnosed with primary pulmonary hypertension and thought her life was over. Cheryl credits her RN Tammy Coplien and the rest of her care team with not only saving her life, but greatly improving the quality of it. Read Cheryl’s story Or take Michael, a Green Lake County Family Care member with a developmental disability. His care team used bowling to ease his transition to Family Care and help keep him on track. Read Michael’s story Care Wisconsin is a nonprofit community organization that specializes in public health and long-term care programs and services for frail elderly and adults with disabilities. Our care management programs serve over 4,100 members living throughout south central Wisconsin along with their families and guardians.
