Transportation Driver
1 Full-Time (1.0 FTE) position (M-F, 7:30am-4:00pm)

The transportation Driver is responsible for providing transportation services to Care Wisconsin participants. This includes safely escorting participants from the vehicle to place of destination as well as assisting with participant positioning on the vehicle. This position also assists in vehicle maintenance, delivering medications and durable medical equipment, and other duties as assigned.

Requirements:

  • High School Diploma.
  • Minimum of one year driving experience in a large van or small bus.
  • Ability to lift 25 to 5 pounds and support a participant’s weight while providing transfers.
  • Ability to be attentive to needs and changes of the elderly population.

Preferred:

  • Experience driving medium duty vehicle in all road/weather conditions.
  • Previous work experience with the use of wheelchair lifts, securement systems and basic first aid.
  • Knowledge of Metropolitan Madison and Dane County area.

Medication Technician 

1 Full-Time (1.0 FTE) position (M-F, 8:00am-4:30pm)

This position is responsible for providing technical support to the LPN’s by assisting with the paperwork, entering orders into the computer and reprocessing and repacking medications for participant delivery.

Requirements:

  • Minimum of a High School Diploma or equivalent education.
  • Certified Nursing Assistant, Medical Assistant certification, or Pharmacy Technician training or equivalent health related experience.
  • Minimum one year health care related experience.
  • Strong commitment to customer service and maintaining accuracy.

Outreach and Enrollment Receptionist- Tri-County and Waukesha County

Full-Time (1.0 FTE) positions (M-F, 8:00am-4:30pm)

This position will provide telephone customer service and clerical support to the Outreach and Enrollment Department by serving as the main contact for inquiries, describing programs, triaging to outside sources, recording initial inquiry and referral information and conveying information to OES staff/other internal staff for follow-up including scheduling home visits for staff.

Requirements:

  • High School Diploma.
  • Minimum of two years administrative support experience.
  • Knowledge of computer applications (Microsoft Office Suite).
  • Excellent customer service and phone skills.

Outreach and Enrollment Representative- Tri-County and Waukesha County

Full-Time (1.0 FTE) positions (M-F, 8:00am-4:30pm)

This position is responsible for initial intake activities, referral set-up and enrollment processing, and completion of initial and ongoing financial eligibility certification for participants and their families/caregivers/case managers into Care Wisconsin’s programs.

Requirements:

  • Bachelor’s degree in Social Work, Human Services or related field or four years equivalent experience.
  • Minimum of two years experience in a health care, long-term care services, or health insurance setting.
  • Experience with computer applications as used in the analysis of data, writing, and presentation.
  • Ability to solve financial eligibility issues related to Medicaid, Medicare, SSI eligibility and referrals.

Preferred:

  • Knowledge of county health care system and aging resources.
  • Knowledge of Social Security Benefits.
  • Knowledge of terms and codes specific to the health care and health insurance industries.
  • Experience in intake, performing non-clinical assessments, and social service agencies or member services.

Service Coordinators- Partnership

1 Full-Time (1.0 FTE) position (M-F, 8:00am-4:30pm)- Dane County

Under the general supervision of the Chief Medical Officer and the day-to-day leadership of the assigned Partnership Team Leader, the Service Coordinators are responsible for general clerical duties in support of the Partnership Program. The Service Coordinator works directly with the Partnership teams to coordinate services and maintains a cooperative and supportive work relationship with participants, family, and staff. Responsibilities will include scheduling participants’ internal and external appointments, providing telephone support, and maintaining participant clinical records.

Requirements:

  • Minimum of a High School Diploma.
  • Knowledge of medical terminology.
  • Experience in the use of computer applications, word processing and spreadsheets.
  • Minimum of one year Service Coordinator related experience.
  • Effective general clerical and customer service skills.
  • Ability to assist in development and implementation of expansion of partnership model in offsite locations.
  • Commitment to providing care for the elderly and the ability to work effectively in a team environment.

Preferred:

  • Health care setting experience.
  • Microsoft Suite (Word, Excel).
