What is Family Care?
Family Care is a state-funded public health program that helps frail seniors and adults with disabilities manage their long-term care needs. This can include transportation, day programs, in-home personal care services, and much more.
How Do I Contact My Care Team?
Check your county’s page or the Family Care Member Handbook to find contact info for your local Care Wisconsin office. Your Care Team can also send you a letter with their direct contact info.

Do I Qualify For Family Care?
Family Care is available to those who meet the functional and financial eligibility requirements.
- Age 18 or older with physical, intellectual, or developmental disability
- Frail adults age 65 or older
- Meets functional eligibility needs, as determined by the Wisconsin Adult Long Term Care Functional Screen
- Financially eligible for Medicaid
- Live in the Family Care service area

Partnership is a publicly-funded public health program that helps frail seniors and adults with disabilities manage their acute and long-term care needs. This can include primary care and other medical appointments, dental services, prescription drug coverage, and much more.

What Is Medicaid SSI?
Medicaid SSI is a state-funded public health program that helps adults with disabilities manage their acute health care needs. This can include primary care, mental health services, medical supplies and equipment, and more.
How Do I Contact My Care Team?
To contact your Care Coordinator, call the Member Care Line at 1-855-463-0026.
Do I Qualify For Medicaid SSI?
Medicaid SSI is available to those who meet the functional and financial eligibility requirements.
- Age 19 or older with a disability
- Receive Medicaid SSI or SSI-related Medicaid
- Not live in a nursing home or other institution
- Not participate in a home and community-based waiver program, like Family Care or IRIS
- Live in the Medicaid SSI service area

Is Medicaid SSI Offered In My County?
Care Wisconsin offers Medicaid SSI in 37 counties across the state. Check your county’s page to see if Medicaid SSI is available where you are.

To contact your Care Coordinator, call the 24-hour Care Wisconsin Member Care Line at 1-855-463-0026.
For transportation, call the Wisconsin DHS transportation manager at 1-866-907-1493 (TTY: 1-800-855-2880).
If you are not yet a member of Medicaid SSI and want information about joining, call the Wisconsin DHS’ HMO enrollment phone line at 1-800-291-2002. Hours of operation are 7:00 am to 6:00 pm, Monday through Friday.

What Is Medicare Dual Advantage?
Care Wisconsin Medicare Dual Advantage (HMO SNP) is a Medicare Advantage program for those with both Medicare and Medicaid. The plan covers all Medicare services and provides extra help when you need it most.
Do I Qualify For Medicare Dual Advantage?
Care Wisconsin Medicare Dual Advantage is available to current members of Care Wisconsin’s Family Care or Medicaid SSI programs who are eligible for Medicare Parts A and B.
This program is also available to individuals not currently enrolled in Care Wisconsin’s Family Care or Medicaid SSI programs who:
- Are eligible for Medicare (Parts A and B) and Full Medicaid Benefits
- Live in the Care Wisconsin Medicare Dual Advantage service area
