​Caring Starts with Long Term Care Supports

Care Wisconsin offers Family Care in 44 Wisconsin counties. The Family Care program helps frail elders and adults with disabilities manage their home and community-based long-term care needs. Our members come to Care Wisconsin after visiting their local ADRC for a needs assessment and options counseling.

As with all our programs, Family Care members are at the center of their care plans and work with care management staff to create goals and paths to success.
