Caring Starts with a Dedicated Team
Care Wisconsin puts you, our member, and your choices at the center of the care plan. From there, many members choose to have close friends and/or family serve as part of the care team. These “natural” or community team members provide another layer of support in addition to our staff.
Care Wisconsin’s experienced Family Care team includes a Care Manager and RN Care Manager who serve as primary contacts and care plan managers. These Care Managers have the support of a variety of on-staff experts to consult as well, including a Long Term Care Director and behavioral health specialists. Our member-centered team approach means members are fully supported.
As a Family Care member, you may also choose to participate in our Self-Directed Supports (SDS) program. SDS offers an additional level of control for members who want to hire employees and direct their own services. Some commonly self-directed services include meal preparation, transportation, job coaching, and housekeeping to name just a few.
