Who Can Join Family Care?
The Family Care program is available to adults with disabilities and frail elders who qualify for long-term care services through Medicaid. There are some other eligibility factors as well including: income and assets limits, functional screen results, and county of residency.
The Department of Health Services’ (DHS) gives a nice overview of this publicly-funded program. There are also helpful quick links on the left. You can see which counties operate Family Care programs in Wisconsin.
If you think that you or someone you care for might qualify for Family Care benefits, the next step is to visit your local Aging and Disability Resource Center (ADRC). There you can meet with qualified, unbiased staff that take the time to understand your unique situation and can then present options from which to choose.
Care Wisconsin’s Family Care program is designed to support the State’s overarching program goals including: prioritizing member choice and providing access to quality services in a cost-effective, sustainable way.
How Do I Join?
When you visit the ADRC, a staff member will give you a list of options and information about the Family Care program as well as your choice in MCO. You do not need to make your choice immediately; you and/or your loved ones can evaluate the information and choose on your time. Our Customer Service team can answer general Family Care program questions and assist with provider searches. Please be aware that we cannot discuss specific health needs unless you have selected us as your MCO.
