Care Coordinators Are Here to Help

Care coordination is one of the many benefits you’ll receive as a Care Wisconsin member. Our Medicaid SSI program experts work with you to create a care plan. We take the time to understand your goals and your unique situation.

Our priority is to provide people with disabilities with high-quality mental, physical and behavioral support.

This means getting you access to the doctors and treatments you need. It can also mean helping you with other things. For example, care coordinators help you:

  • Arrange transportation to and from qualified medical or therapy appointments
  • Obtain eye care/glasses, hearing aids, and personal care supplies, and
  • Access mental health and/or substance abuse treatment programs
  • Make appointments or reminders for you, and
  • Address any food or housing issues you may have

We also have a 24-hour free nurse call line.

Rely on our Care Coordinators as little or as much as you need to meet your care plan goals. Our team is here for you.
