​Caring Starts with the Coverage You Need

Medicare Dual Advantage (HMO SNP) is a Medicare Advantage plan for people who qualify for both Medicaid and Medicare. The plan gives you access to:

  • Hospital stays and doctor visits (Medicare Part A & B)
  • Prescription drug coverage (known as Medicare Part D)
  • 24-hour Nurse Care Call Line, and
  • Experienced Care Coordinators who can help schedule appointments and connect you with resources

If you receive Medicaid benefits and qualify for Medicare because of a disability determination or your age (65 and older), you may be eligible for our Medicare Dual Advantage Plan. Our plan members pay no monthly premiums and no plan deductibles.
