Who Can Join?

People who have full Medicaid and Medicare (Part A & B) benefits are usually eligible to join a Medicare Advantage Special Needs Plan. Care Wisconsin’s plan is called Medicare Dual Advantage (HMO SNP). To be a member of Care Wisconsin’s plan, you must meet Medicare and Medicaid eligibility and you must also live in one of the 31 counties that we offer the plan. We have several licensed Medicare product advisors on staff to help you determine if Medicare Dual Advantage is right for you. Call our toll-free number for a free, no-obligation consultation. We will help you:

  1. Find out if you may qualify for Medicare and Medicaid
  2. Review the provider and pharmacy directory and list of covered drugs, then
  3. Sign up for Dual Advantage if you think it’s right for you

You do not have to be an existing Care Wisconsin member to qualify for our Medicare Dual Advantage plan.

If you are already a member of our Family Care or Medicaid SSI programs, you may meet the eligibility requirements for our Medicare Dual Advantage program. When you are a member of both programs, we can more easily coordinate your health and long-term care benefits.

How Do I Join?

You can join Medicare Dual Advantage by contacting us directly. The enrollment process is fairly easy and we’re here to help you at every step.

You can contact us by telephone, email, or mail to set up a no-obligation consultation. We have several licensed Medicare agents on staff who can explain the Care Wisconsin Medicare Dual Advantage plan.

This consultation can happen over the phone or in-person, by appointment. To make sure the plan will fit your needs, we will review your current medications and healthcare providers.

If the plan is right for you and you choose to enroll, you can fill out this application and return it to us.

Individuals who qualify for Medicare Dual Advantage can enroll at any time during the year.

*Care Wisconsin’s Medicare Dual Advantage is an HMO SNP with a Medicare contract and a contract with Wisconsin’s Medicaid program. Enrollment in the plan depends on contract renewal with these entities. This information is not a complete description of benefits. Benefits may change January 1 of each year. You must continue to pay your Part B premium unless it is paid by Medicaid. Co-pay amounts vary based on the amount of Extra Help you receive.
