Caring Starts with Member-Centered Care
Care Wisconsin puts you, our member, and your choices at the center of the care plan. From there, many members choose to have close friends and/or family serve as part of the care team. These “natural” or community team members provide another layer of support in addition to our staff.
Care Wisconsin’s experienced Partnership team includes:
- A Care Manager and RN Care Manager who serve as primary contacts and care plan managers
- A team of Administrative Assistants to help with scheduling appointments and services
- A highly-credentialed team that can be called on as needed. This includes a medical director, pharmacists, nurse practitioners and other subject matter experts in mental and behavioral health
As a Partnership member, you may also choose to participate in our Self-Directed Supports (SDS) program. SDS offers an additional level of control for members who want to hire employees and direct their own services. Some commonly self-directed services include meal preparation, transportation, job coaching, and housekeeping, to name just a few.
