Care Wisconsin Partnership-Health and Long Term Care

Who Can Join?

The Partnership program is available to individuals 18 and older with intellectual, physical or developmental disabilities, who qualify for nursing home level of care. Additional eligibility factors include income and assets, functional screen, and county of residency.

If you think that you or someone you care for might qualify for Partnership benefits, the next step would be to visit your local Aging and Disability Resource Center (ADRC). There you can meet with qualified, unbiased staff who take the time to understand your unique situation and can then present options from which to choose.

A key word here is choice. DHS stresses that member choice is the “cornerstone” of the Partnership program and that is true from your initial visit to the ADRC to your relationship with a managed care organization like Care Wisconsin.

How Do I Join?

When you visit the ADRC, a staff member will give you a list of options and information about the Partnership program as well as your choice in MCO. You do not need to make your choice immediately; you and/or your loved ones can evaluate the information and choose on your time. The DHS website lists the contact information for each MCO if you have questions for them. If you would like to call us, we are available toll free 1-800-963-0035 (TTY users should call Wisconsin Relay 711).

Our Customer Service team can answer general Partnership program questions and assist with provider searches. Please be aware that we cannot discuss specific health needs unless or until you have selected us as your MCO.
