Caring Starts with Access to Medicines You Need

Many of our members need access to a variety of medicines and therapies to help them maintain or improve their health.

Two of Care Wisconsin’s programs include a Medicare prescription benefit, also known as Medicare Part D. Our eligible Partnership members as well as our Medicare Dual Advantage members use their Part D benefit to access the medicines they need.

Members and providers can access the most up-to-date list of covered drugs, also called the Formulary, on the individual program pages or under the tabs below. You can use our online Pharmacy Directory tool to find a location that’s most convenient.

If you have questions about your pharmacy benefits, don’t hesitate to call toll free 1-800-963-0035 (TTY users should call Wisconsin Relay 711). As always, if you need help in another language, please let us know.

If you have questions directly for the Centers for Medicare Services related to your Part D plan, we’ve listed CMS resource numbers on our More Resources page. You can also visit CMS’ general Medicare Part D webpage.

Pharmacy and covered drug information is listed below under each program.
